About 45 days prior to your construction supervisor license expiration date, the state will send you a renewal form via US Mail. Your continuing education may be completed at any time; however, you may NOT submit for renewal until you have received this renewal form.
If you are within the 45 day window of your expiration and have not received your renewal form, it may have gotten lost in the mail. You may request for a new renewal form to be sent using the link below. You can either email it or mail it; just follow the instructions on the form:
Once you have that renewal form, you just need to mail it into the state (address is on the form) along with your continuing education certificates. All instructions are on that renewal form.
